Getting to the office

We are at 1705 Bardstown Rd, Louisville, KY 40205. Near the corner of Bardstown Road and Eastern Parkway.

Bardstown Road is busy, and the building is easy to drive past the first time. If you are coming for a first appointment, give yourself a few extra minutes, or call when you are close and we will talk you in.

Before you send this

Please do not include Social Security numbers, tax documents, bank or account numbers, or images of your ID in this form or in regular email. Send anything sensitive through the office's approved secure method, or bring it to your appointment.

This is an ordinary contact form on an ordinary website. Treat it the way you would treat a postcard, not a locked filing cabinet.

If it is urgent

If a deadline is close, or you have received a notice with a response date on it, please call rather than using this form. Messages are not monitored around the clock, and a notice with a short window is exactly the wrong thing to leave sitting in an inbox.
